4. Outline briefly what a visitor can expect from a one-day, one-week and one-month visit to the department(s) (ESCMID Observership programme)

General presentation of the laboratory and its facilities. Opportunity to speak to several of the staff, including infection control nurses, attending the Intensive Care consultation rounds. For one week

Introduction to the organisational structure from hospital to lab. Introduction to the Quality System (ISO 15189), as well as to the Visitation System of the Medical Society. Introduction to principles and practice of infection prevention by Selective Digestive tract Decontamination (SDD) as well as to the Infection Control policies (e.g. Search and Destroy for MRSA containment). The cooperation with the local and national public health sector is discussed. The ESCMID observer gives a presentation of himself and his parent department, including special areas of expertise, as well as the national health care system, etc. For one month

14. Description of the department/institution and profile of competence

The Laboratorium voor Medische Microbiologie & Infectieziekten (LMMI) is part of the large teaching hospital, Isala klinieken. It is a model representative of the Integrated Clinical Microbiology approach. The ISO 15189 accredited LMMI has about 40 employees, including a medical molecular microbiologist (PhD) and three clinical microbiologists (MD/PhD). It serves the hospital, family physicians and nursing homes in the area, also working closely together with the municipal Public Health Dept.. It processes about 110,000 samples/yr in the Bacteriology, Parasitology, Mycology, Serology and Molecular Microbiology lab departments. There is quite some expertise in the principles and practice of infection prevention in Haematology and Intensive Care, as well as in containment of MRSA ("search and destroy" approach).
The Isala klinieken has about 1000 beds, 3900 employees, 230 consultants, about 30 medical specialties, medical facilities ranging from three intensive care units (general, cardiothoracic, neonatal), to haematology transplant unit, IVF center, haemodialysis unit, cardiothoracic surgery, neurosurgery, regional trauma center, radiotherapy center.
The city of Zwolle lies in the center/east in The Netherlands, in a less densely populated area, surrounded by a beautiful and charming environment. It is a regional center with about 120.000 inhabitants, and growing fast.

The laboratory services the hospital as well as the family practitioners and nursing homes in the area. A significant amount of the population is not vaccinated for religious reasons, which provides epidemics of vaccination-preventable diseases such as measles, mumps, etc.
